If you have a concern about the care you received or the operation of our clinic, please complete the Patient Complaint Form. You may also need to fill out a Health Information Authorization Form.
All complaints, whether written or verbal, will be investigated. You can expect a response within 10 business days. If the complaint involves actual or potential harm, it will be addressed immediately.
If you are not satisfied with the resolution or feel that your complaint was not addressed in a timely manner, you have the right to contact the
Patient Ombudsman under the Excellent Care for All Act, 2010:
